Let me attempt to clarify (this might be a big mistake).
My answer to the original question is not out because the fielder is not in control of his movement as he falls and then the ball has touched the ground (whether the ball is still resting in the fielder's hand or not at that stage is irrelevant).
As for the Kiwi catch, I think it was out because he touches the ball to the ground only when he stops sliding and is getting up off the ground by which time he is in control of his movements and the movements of the ball.
I think the 'blade of grass poking through the fingers' question can be answered simply as follows:
I do not believe that a blade of grass poking through the fingers to touch the ball is detectable and if you cant detect it, then it didn't touch the ground and hence it is out. However, if you take that question and read the rule, then it is clearly a not out decision for me.
